Faculty Activity Summary
Name: ______________________________
Please electronically submit as three separate documents your faculty activity summary, the narrative on your professional goals, and a current curriculum vitae no later than Wednesday, February 3, 2016 to Marsha Crowe (marsha.crowe@centre.edu) and to your division chair. If you hold an endowed professorship, please take this time to also attach an annual report of your activities using those funds. Naming your documents last name.firstnameCV.doc, (e.g. crowe.marshaCV.doc), last name.firstnameFAS.doc, and last name.firstnameGoals.doc would be really helpful. Sending your FAS as a word document is helpful to your division chair and the associate dean as they copy and paste from your FAS to prepare for your annual review.
Periodically, we are asked for cv’s by individuals at the College considering faculty members for awards and honors, or as information on faculty grant proposals.
£ Please check here if we have your permission to share your cv for these college-related reasons. Note: NOT checking this box may mean you are ineligible for some honors.
